Help with icon creation

Since i have to create some custom icons for an accounting software, I would be happy if someone helped me out explaining few things looking at this image.
1) and 2) - How exactly are these curls top-right created? Is the rectangle and the curl in separate layers?
(there were no good tutorials on the web)
3) How are these folders created, as they look like one folded peace and the folder has a transparent front cover (4).

the first to icons use two layers, sometimes more, u have to use layer options in phtoshop, and use gradients, the tutorials should explain that
for the folder, it looks like they used 3 layers, 1 the white part of the folder 2 the text, and 3 the blue part was created, and then the opacity(transparency) was turned down a little, on each of the layers, they once again used layer options to create the border lines for the folder
