This usually does the trick:

<form method="get" action="http://www.google.com/custom">
<input type="submit" name="sa" value="Search">
<input type="text" name="q" size="20" maxlength="255" value="">
<input type="hidden" name="sitesearch" value="YOURDOMAIN.com">
</form>
<small><a href="http://www.google.com/search">Powered by Google</a></small>

If you want to search your domain, alter the YOURDOMAIN.com value above. If you want to search the internet, remove that entire entry.
