About the font size, this is an half page ad in the news paper in that readability is very clear. About the other question, as you said yes "challenge" is the answer. (In the beach you may get tsunami also)

honestly, it says more "church" to me than hospital. maybe it's the way the guy is looking up to the heavens, maybe it's the type of beach scene, dunno. but i don't think this evokes the feeling you're going for.
