Need help on email form for charity webpage

I am pretty bad at this stuff. I got some help in creating a waiver form for a charity event that can be emailed to my email account.

It is functional but it does not email me the entire agreement. I just emails me information like name, age, sex, location etc.

Do I need to set some code that it send the whole agreement.

what function picks what information is sent my email box?
