Stupid Question about submitting a form from my website—HELP!!

I have a silly, embarrassing question. I have created a simple, succinct questionnaire for my eBay clientele to fill out directly from my website (see link below so you can see what I mean) and I am trying to figure out, once they click the "submit" button, where does it go? I can't seem to figure out how to get their filled out questionnaire with their answers sent directly to me—either via e-mail or whatever.
I hope I'm explaining this right, but I don't know any other way to explain it. I tried doing a few test runs by filling out the form myself, but obviously, it's just going out there into dead web space. It doesn't come to me or anything.
I know this is a ridiculous question for a designer to ask, but I will be the first to admit that I'm a geek when it comes to web design & HTML. If anybody can help, I would really appreciate it.

So you created this form in an html application I suppose and now you want the form to work, right?
Basically you need a back-end, an engine that accepts the entry and sends it to your email or stores in a database. If you have a programmer he can do it easily for you. If not you may want to use one of the online questionnaire systems, but that would mean you have to use their interface.
No, unfortunately I do not have a programmer. So this isn't anything I can do myself, I take it?? What sort of "engine" do you suggest? Sorry, I have never heard of this sort of thing, so I guess I don't know what it means, yet.

& download their generously offered free script. It's very simple to configure & you'll have your forms working in no time.

Hello! I tried to download the free program, and this is the error message I received:

I tried doing this in both Safari & Firefox. I know sometimes Safari can have issues, so I tried Firefox and I still received the same error message.
Do you know what this means???
